What in the world is a Meds Info card?

     That's simple. It is a plastic card with your name, medical procedures and diagnoses you have had on one side and medications you are currently taking on the other. I came up with this idea one day when my mother had to take my father to the emergency room. My father is fighting a battle with cancer and was in a lot of pain when I met them at the emergency room. The first questions that were asked upon arrival were what procedures have you had, when were they and what medications are you currently on. My mother was trying to remember all the information they requested. After finding a crumpled piece of paper in her purse with his medication written on it, she had most of the information they wanted. It seemed to me that for someone that has medical problems, or takes very many medications, it would be nice to have all the information in one place where it is easy to find.

How To Get One

    The preferred method is to download the Microsoft Word form by clicking on the link below. Fill out the form (use tab to move from field to field, use enter to add a second line in a box) save it with a new name and e-mail the form to rich@vridcards.com (include your mailing address, phone number, how many copies you would like to have and how you would like to pay). Rich will print the number of cards you request and as soon as payment is received, send them to you. If you have problems with the form or would prefer not to use it, you can include the information in an e-mail and send it to rich@vridcards.com.

    Once the information is received and the card(s) are printed, you will be contacted with regard to your payment preference. Meds Info Cards are $5.00 each, including shipping and handling. If you prefer to pay by check, the cards will be held until the check clears. If you prefer to pay by credit card (VR ID Cards accepts Visa and Master Card), the transaction will be processed via phone before your order is sent. Once payment is received, your cards and receipt will go out USPS first class in a business envelope.

    Once your order is completed and mailed, all medical information will be deleted from our system. VR ID Cards does not maintain or share any medical information on Meds Info Cards customers.
